Shockwave intravascular lithotripsy as a novel strategy for balloon undilatable heavily calcified chronic total occlusion lesions

چکیده
Background: The successful percutaneous coronary intervention (PCI) in chronic total occlusion (CTO) improves the long-term outcome patients with artery disease (CAD). Heavy calcification remains one of strongest predictors an unfavorable PCI. In this case series study, shockwave intravascular lithotripsy (S-IVL)-a novel balloon-based system facilitating modification calcified lesions was evaluated. Methods: study population consisted five heavily calcified, undilatable-CTOs treated S-IVL selected out all consecutive CTO-PCI performed at two high-volume cardiac centers. Results: registry included 5 CTO — procedures average J-CTO 2.6 points. short-term follow-up period, including first 30 days, no cases acute in-stent thrombosis, target lesion failure, or major adverse and cerebrovascular events were noted. Conclusions: present data suggest that approach can be safe useful treatment complex lesions.

A 33-year-old woman with a 3-year history of idiopathic CCP and recurrent attacks of acute pancreatitis was referred to our institution for PD stones management. Prior attempts of endoscopic retrograde cholangiopancreatography (ERCP) approach had failed. Plain abdominal X ray showed calcified stones in the pancreatic area (Fig. 1A). Magnetic resonance cholangiopancreatography (MRCP) demonstrate...
متن کاملA noncompliant, high pressure balloon to manage undilatable coronary lesions.
Undilatable coronary lesions, due to coronary calcification, are encountered with increasing frequency. Percutaneous intervention of such lesions comes with the risk of stent underexpansion, which increases the risk of stent thrombosis and restenosis. A variety of techniques and devices are available to assist the interventionist in the treatment of calcified lesions. The OPN NC (SIS-Medical AG...

We present a 77-year-old female with heavily calcified chronic total occlusions (CTO) in a superficial femoral artery treated by endovascular therapy using a Wingman crossing catheter, which is an over-the-wire catheter with a metallic blade, controlled manually. The blade could probe and track the calcified cap of CTO, wherein any hydrophilic guidewires or looped wires could not penetrate. Mor...
متن کاملIntravascular Ultrasound Guided Percutaneous Coronary Intervention for Chronic Total Occlusion
Chronic total occlusion (CTO), a fascinating and dynamic niche in the realm of coronary artery disease, represents a major technical challenge for interventional cardiologists despite evolution of better guidewires, devices, experience and techniques. Effective wiring technique is the corner stone to success of percutaneous coronary intervention (PCI) in CTO. As a guide for guidewire crossing i...
متن کاملIntravascular Ultrasound and Angiographic Predictors of In-Stent Restenosis of Chronic Total Occlusion Lesions
Despite the benefits of successful percutaneous coronary interventions (PCIs) for chronic total occlusion (CTO) lesions, PCIs of CTO lesions still carry a high rate of adverse events, including in-stent restenosis (ISR). Because previous reports have not specifically investigated the intravascular ultrasound (IVUS) predictors of ISR in CTO lesions, we focused on these predictors. We included 12...
